DIVE TRAINING SCHOLARSHIP


Scuba Made Easy, LLC offers a scholarship awarded to any person with disabilities who wishes to pursue scuba diving education.  Candidates can be male or female, must be able to obtain a physician’s approval to scuba dive, and can be at least 12 years old.  This scholarship is to be used towards dive education at a Scuba Made Easy, LLC training facility.

All applicant materials must be received electronically and must be submitted together as a single submission. The application packet must include:

A)     250 word essay written by the applicant including

·         Your name, address, phone number, email address, list of hobbies, and volunteer activities (if applicable),

·         Description of why you think scuba diving would be important for you.

B)      One letter of recommendation with the person’s name, address, phone number and email address included.

In order for your application to be processed, you must provide at least one means of contacting you. Incomplete applications will NOT be accepted and will NOT be considered.

Biographies and letters of recommendation should be sent electronically as "Attachments". These documents must contain file extensions and be compatible with a Windows platform! Letters and biographies can be sent as TEXT (.txt), WORD (.doc) or Acrobat (.pdf) files. If these are currently in hardcopy format, then you must convert them. We recommend using a scanner to reproduce the pages at 150 dpi (screen resolution) in black and white and saved them as JPG (.jpg) or TIFF (.tif) or Acrobat (.pdf) files.

Please maintain a copy of your completed nomination and all supporting materials. You should receive an acknowledgement of your application within one week.

Award winner (or parent / legal guardian) must be willing to sign a waiver stating that they agree to allow scholarship award recipients name, application and award photograph to be released to news outlets.  The ONLY purpose of this release authorization is to enhance the awareness of people with disabilities and organizations that provide support to the disabled that programs of this nature are available.

NOTE: Applications become the property of Scuba Made Easy, LLC and will not be returned.  All provided information (including postal/email addresses, phone numbers, etc.) remains confidential and will be used only for correspondence relating to this application.
